Some health conditions need a number of health experts to work together. We are privileged to have a team of amazing health professionals under the one roof! Together we can help you manage more complex needs. Our fully qualified Exercise Physiologist, Physiotherapist, and Podiatrists work closely with clients to create programs specifically tailored to individual needs. Whether it’s a short burst of care or care for a longer time to achieve your goals, our team will put a plan together to help you now and into the future.
Our friendly practitioners are passionate about an "Active Over Passive" approach.
We believe in helping our clients to become more independent. By becoming more reliant on your own body, you will require less external assistance for your day to day living.
By using an active strength program tailored to your specific needs, our exercise specialists will guide you through a program designed to progress your capabilities at a pace which works for you and your body.
You will first attend a full Initial Consult performed by your chosen therapist. This will include:
After your initial assessment we will set up an NDIS Treatment Planning Consultation (NTPC). Yourself, your representative or practitioner and our practice manager will discuss your needs and propose a service agreement. Every NTPC is approached individually. We aim to ensure that you find a treatment plan that works within your specific needs and current planning.
Reporting is an important requirement in NDIS services, our practitioners are able to work with third parties, or other practitioners in your team. These may include: external plan managers, finance companies or occupational therapists, in order to take a cohesive approach to your treatment.
At Boost Health, all our NDIS services are compliant with government guidelines, billings and terms and conditions and are reviewed and updated regularly.
At the end of your NDIS plan, your practitioner will sit down with you to review and discuss your progress. They will also chat with you about what else you would like to achieve. After this your practitioner will write up your progress report and add their recommendations about how you should best proceed from here and reaching the next step in your goals?
