Fungal nail infections are also known as Onychomycosis or Tinea Unguium. It is a common condition where the nail becomes thick, adopts a white or yellowish discolouration, and can also become separated from the nail bed. This is due to fungal growth.
Fungal toenails can affect individuals of all age groups but are especially common in older adults, where the nails can become cracked and brittle, allowing pathogens to enter the toenail. These infections can be incredibly stubborn and some cases will take weeks, months, or even years to resolve.
Common signs of fungal toenail infections:
- Thickening of toenails
- Yellow/white discolouration
- Distortion in the shape and growth
- Malodorous
- Brittle and dry toenails
Why do they occur?
- Being barefoot in communal areas where pathogens are more likely to be present (including gyms, swimming pools, change rooms, sports clubs)
- Poor hygiene practices – not washing socks regularly
- Occupation
- Trauma to the nail, resulting in the nail becoming frail or falling off
- Reduced circulation to the feet
- Weakened immune system
- Side effects of certain medications
- Sweating heavily
- Diabetes
What can a podiatrist do to help you?
- Nail and skin assessment
- Manual treatment on the nail – this involves reducing the bulk of the nail through mechanical debridement. It can also include cutting the nail down and clearing out the sulcus.
- PACT laser treatment – Photodynamic Antimicrobial Therapy is a form of treatment that kills off fungal cells without causing any pain or discomfort. The laser targets the fungal cell walls through the application of a fungal nail gel, which creates a sensitivity reaction. These cells are then destroyed upon exposure as a result of a chemical reaction, leaving the healthy cells unaffected.
- Recommendation of fungal nail ointments and solutions
- Natural antifungal remedies such as tea tree oil and methylated spirits
- Referral to GP for oral antifungal medication if there has been no improvement with the strategies listed above.
Tips to prevent re infection:
- Apply a topical antifungal solution daily
- Wear shoes and socks made of breathable materials
- Change socks daily
- Wear moisture absorbing socks
- Wear thongs in communal areas
- Use a separate towel to dry your feet
- Disinfect the shower floor regularly
